What is the SCN Network?

The Social Care Network and Health-Related Social Needs Program in New York State
are regional systems created under the NY Health Equity Reform (NYHER) 1115 Medicaid Waiver to better integrate health care with social care for Medicaid members. SCNs bring together health plans, providers, and community-based organizations to identify and address individuals’ health-related social needs (HRSNs) non-medical factors such as housing instability, food insecurity, transportation barriers, utility issues, and other social or economic challenges that significantly impact health and well-being.

Under the program, all Medicaid members can be screened annually for HRSNs using a standardized tool, and those with unmet needs can be connected to support services. Some eligible members may receive enhanced HRSN services like nutrition support, housing assistance, care management, and transportation. The goal is to reduce health disparities and improve outcomes by addressing the social factors that shape health.
